Insurance Card
Picture ID
All current medications (prescripton and over-the-counter)
Referral (if needed)
VERY IMPORTANT: Primary Care Physician information is needed for two important reasons:
- to process insurance claims
- to send a summary of your evaluation and recommended treatment to your physician
- Primary Care Physician's Address, Phone and Fax numbers
*Failure to bring the above necessary items will result in the rescheduling of your appointment.
