Appointment and Scheduling Policies
Appointments:
If this is your first visit, please make sure you have all required paper work.
see "What to Bring on Your First Visit"
Arrive 15 minutes early to complete any additional paperwork.
If you are later than 15 minutes your appointment will be rescheduled
note: It is very important to keep both Primary Care Physician and Insurance information
up-to-date.
If either of these have changed, please bring the new information with you.
Rescheduling:
We understand that things change and appointments can't always be kept.
If you need to reschedule, kindly give us at least 24 hours notice.
Cancellations without notice will be charged a cancellation fee not payable by insurance.
To reschedule appointments for either office call (904) 378-2880.
For after-office hours you may leave a message with our answering service.
